They are accessible online for your convenience so that you may complete them before your first appointment. This paperwork is very important to providing you with the best and most appropriate form of treatment. If you have any questions while completing your intake paperwork, please call or email your therapist and they would be happy to assist you with the intake paperwork.

Every client at Family Service Center needs to review the Privacy Practices and Client’s Rights and Responsibilities packet. While you do not need to print these forms, we encourage you to keep them for your records.
